I understand that all personal and pet-related information provided in this form will be collected, used, and stored by PetTransPRO Philippines for the sole purpose of facilitating pet transportation services. PetTransPRO Philippines will handle my information in compliance with the Data Privacy Act of 2012 (Republic Act No. 10173) and other applicable laws on data protection.

Consent to Share Information
I consent to the sharing of the information provided in this form with PetTransPRO Philippines, its employees, authorized representatives, airline partners, animal facilities, veterinary authorities, and other third parties as required to process my pet’s relocation. I understand that such sharing is necessary to ensure compliance with international regulations and facilitate the smooth coordination of my pet’s travel.

Accuracy of Information
I affirm that all the information I provide in this form is accurate and truthful to the best of my knowledge. I understand that any inaccuracies or omissions may result in delays, additional costs, or complications in the transportation process, for which PetTransPRO Philippines will not be held liable.

Medical Declaration and Liability Waiver
I declare that my pet is in a fit condition to travel and that I have disclosed all known medical conditions, including but not limited to respiratory issues, heart conditions, anxiety disorders, and any other health concerns that may affect my pet’s travel. I understand that failure to disclose such medical conditions may result in health complications during transit.

I acknowledge that PetTransPRO Philippines, its employees, and its partners are not liable for any health issues, injuries, or fatalities arising from pre-existing, undisclosed, or aggravated medical conditions during transportation. I also understand that Philippine laws, including the Animal Welfare Act (Republic Act No. 8485 as amended by Republic Act No. 10631), require pet owners to act responsibly in ensuring their pet’s well-being before subjecting them to travel. I assume full responsibility for any medical concerns that arise as a result of my pet’s transportation.

Acknowledgment of Risk
I acknowledge that while PetTransPRO Philippines will exercise utmost care in handling my pet’s relocation, certain risks may arise due to factors beyond their control, such as airline delays, customs inspections, or quarantine requirements. I agree that PetTransPRO Philippines will not be held responsible for such events.

Agreement to Terms
By checking this box and proceeding to fill out the form, I confirm that I have read, understood, and agreed to the terms stated in this waiver. I also acknowledge that I have the right to withdraw my consent at any time by notifying PetTransPRO Philippines in writing.
